I am considering a breast reduction?

And I am 30 years old with three children. Normally I'm a 36D (which I hate, especially when I'm trying to find a t-shirt) and a healthy weight of about 135lbs to 140lbs at 5ft 6inches. I live in Alberta, specifically the Edmonton-St.Albert area and I want a smaller bust-line. My bust hasn't caused me too much trouble aside from some rather permanent dents in my shoulders where my bra straps have been. I'm aware my request would be considered "cosmetic" but I hate the way my bust looks, especially with the way they sag. Who in the area can do that kind of work, how much am I looking at, and what do I have to do to get it?
